The following is a case study of treating combined static and dynamic dimple lines...
I treated a client end of last year and performed correction of nasolabial folds (smile lines) and dimple lines with soft tissue fillers. I was not able to use the cannula technique for her (which in general offered better comfort and less bruising) due to quite significant acne scar tissues. She returned recently as there has been a reduction in the soft tissue fillers and reappearance of the dimple lines.
This is quite unusual, as soft tissue fillers should last 9-12 months depending on the type, volume of fillers and metabolism of the client.
On client's return, it was noted that there were still some fillers around the upper nasolabial folds but some fillers has been metabolized (gone) around the dimple region.
After a complimentary re-treatment with a thicker filler, she returned 4 weeks later and was still dissatisfied in relation to her dimple lines. On examination, the nasolabial folds has been corrected, there has been minimal improvement with the dimple lines, however, the fillers could still be palpated under the skin (i.e. the fillers were still in the skin layer).
Her dimple lines were noticeable at rest, also known as static lines, and more accentuated when the client smiled, also known as dynamic lines.
Static lines are generally more easier to treat than the dynamic lines. The treatments for static and dynamic lines are quite different, often, should be performed on separate occasions.
Thicker fillers should be used to correct the volume loss of the nasolabial folds first. This will correct majority of the static folds. When the client returned for review, the upper nasolabial folds have been completely treated after the first and second sessions. The third part of the treatment would correct the dynamic folds, especially around the dimple region. Unfortunately, the client was due to attend a wedding and decided not to go ahead with further treatment.
Treatment of dynamic folds requires light to medium soft tissue fillers. This is achieved by a special method using the ferning or cross-bridging technique. This will provide better support for these dynamic folds.

Time flies!
I can't believe another year has passed! Many thanks to my patients, clients and clinic owners for their warm Christmas and New Year wishes. Cosmetic clinics session have restarted!! We are now full speed ahead, just in time for Chinese New Year, Year of Rabbit (3/2/2011) and Valentine's day (14/2/2011).
Please remember to allow for 7-10 days for down time prior to any social engagements.
I look forward to seeing you soon.
Christmas Greetings
On behalf of all the clinics, I wish you and your families a very Merry Christmas and a Happy New Year. Please take good care during this festive season and I am looking forward to seeing you all in 2011.
With great regret, I will no longer be available at Lalor Plaza Clinic, South Yarra Caci Clinic and Beauty Profile Eltham Clinic in 2011. I have taken up a permanent student health medical practitioner position at Swinburne University of Technology at Hawthorn Campus. Student health promotion has always been my special interest and passion. In 2011, I will continue my fortnightly clinics at Total Bliss Templestowe and Greensborough (Locations) . I will also cover for Dr Dennis McCurdy at Canterbury Clinic during his absence.
Thank you for all your support this year and it has been a great pleasure working with you and looking after you.
Best Wishes and have a safe holiday,
Dr Louisa
