DISCLAIMER AND REFUND POLICY 2011

Please read this information before signing the CONSENT form for treatments.

1. By signing the consent form, you agree to the following terms and conditions.

2. If you DO NOT agree, please inform Dr Louisa and she will be able to refer you to an alternative cosmetic practitioner.  There will be no charge for this consultation.

3. Dr Louisa Yim will take every effort to ensure:

· up to date information on cosmetic services or medical information

· clients’ satisfaction with full consent of certain treatment and discussion of expected outcome

· clients receive and understand post treatment care plan

· clients may directly contact Dr Louisa Yim by email or telephone for further advice with treatments.

4. Photos:

· The photos have not been "photo-shopped" to mislead the general public.

· These images have only been cropped to ENLARGE the area of interest.

· Consent has been given to release these images and they do not identify any individual in particular.

5. The outcome of any cosmetic treatments is subject to INDIVIDUAL VARIATION.

6. Every effort will be made to ensure client satisfaction but Dr Louisa Yim cannot guarantee exact replica of successful treatments as demonstrated in the photo album or photos provided by you (the client).

7. Should an unsatisfactory outcome occurs, clients MUST contact Dr Louisa Yim or participating clinics via email or telephone as soon as possible (MUST BE within four weeks) of treatment to ensure these issues can be amended as early as possible.

8. Similar to the pharmacy, as cosmetic treatments are S4 Doctors only prescription medicine, NO REFUND will be available once a cosmetic treatment has been provided for the client.

9. As every client responds quite differently to cosmetic treatments, if less than the desired outcome occurs, Dr Louisa will generally provide you with one complimentary similar or repeat treatment to the equal amount of value that you have purchased.

10. Dr Louisa Yim, JustHost and participating clinics are not liable for any adverse outcome due to clients:

· Misunderstanding of post treatment care plan

· Mishandling of products

· Acting against medical advice

· Combining other treatments from another source without notifying Dr Louisa Yim in advance or immediately after treatments

· Lost of profits or earnings as a result of receiving treatments, using this sites or other third party sites

· Unauthorized access to information, errors due to viruses, deletion of files, omission, interruptions, mistakes, communication errors, destruction or theft of the website www.drlouisayim.com
